Sweet Success Story: The Device Education That Tripled Daniel’s Time in Range

When Daniel*, a 50-year-old man with type 2 diabetes, joined his provider’s SweetSpot program in April 2025, he was determined to get his health back on track. In recovery from addiction and beginning a new wellness journey, he was eager to improve his diabetes management — but unsure where to start. Balancing a busy job, family responsibilities, and a history of inconsistent medication use, Daniel’s Time in Range (TIR) was just 19% when we first met.
Despite his interest in making changes, Daniel’s days were hectic. He was open to suggestions right from the start, but he was often working during our calls, so each encounter needed to be very focused.
As a SweetSpot Certified Diabetes Care and Education Specialist (CDCES), I partner closely with providers to offer remote care for their patients using CGMs and insulin pumps. I review patients' diabetes device data every month, suggest any treatment adjustments if needed, and keep in touch with patients to help them stay on track between visits. Watching patients make meaningful progress is the best part of my job — and Daniel’s transformation is one I’ll always remember.
Finding the Hidden Barriers
When he first enrolled in SweetSpot, Daniel’s CGM data showed high post-meal spikes, overnight elevations, and big swings from day to day. At first, I focused on building trust and learning his habits.
Through open-ended, nonjudgmental questions — a hallmark of SweetSpot’s motivational interviewing approach — I uncovered key challenges:
Frequent sugary drinks and high-carb snacks
Skipped or reduced mealtime insulin doses
Inconsistent meal timing
Misunderstanding of his insulin delivery device
Daniel thought his insulin delivery device only provided mealtime insulin. When he ran out of bolus “clicks” for the day, he removed it — not realizing it also delivered his daily basal insulin. Once we discovered that, it explained a lot about his glucose variability.
Starting with Small, Doable Changes
Rather than overwhelm Daniel, I helped him choose one change at a time:
Reducing sugary drinks and high-carb snacks between meals
Matching rapid-acting insulin to carbohydrate intake
Wearing his insulin delivery device for the full 24 hours, as prescribed
Being consistent with meal patterns and pre-meal insulin clicks
We always ask the patient to decide which change to focus on first. That way, the plan is realistic and patient-driven.
From Setbacks to Breakthroughs
By May, Daniel’s TIR had climbed to over 50% — then dipped to 25% again. He was discouraged, believing he was on track. That’s when a deeper dive into his CGM data revealed the insulin delivery device issue.
After educating Daniel on the device’s basal insulin delivery and reinforcing the importance of consistent wear, things turned around quickly. Just three weeks later, his TIR jumped to 62%. Sometimes the breakthrough comes from asking one more question and taking the time to connect the dots.
The Human Impact
While the numbers were impressive, I noticed another important change: Daniel’s confidence.
He’s a great example of someone who truly wanted to improve, but faces a lot of daily challenges. With SweetSpot, he gets ongoing guidance and education that helps him make focused changes — the kind that add up to big results.
Daniel is now more engaged in his care and remains committed to making further progress at each call.
