At SweetSpot, we believe diabetes care doesn’t end when the patient leaves the clinic. For many, the true breakthroughs happen between visits — when patients have someone in their corner to help them make sense of the data, the devices, and their daily routines.
For Jenny, a 60-year-old living with type 1 diabetes, that support made all the difference.
Though she had been living with diabetes since 2018, it wasn’t until recently — when she enrolled in SweetSpot’s CGM/pump monitoring program through her provider — that things started to click. With consistent coaching, education, and a little encouragement, Jenny went from feeling unsure about her insulin pump to confidently managing it independently.
Identifying CGM and Insulin Pump Issues Through Remote Monitoring
Jenny was using both the Omnipod 5 insulin pump and Dexcom G6 CGM. While reviewing her device reports, SweetSpot’s clinical team noticed inconsistent Time in Automated Mode and recurring highs.
Like many patients, Jenny didn’t always use the same terms as her providers when describing what was happening. But that’s where SweetSpot's data monitoring comes in: Our team connected the dots between her terminology and her actual CGM and pump patterns, flagging possible issues with insulin delivery and device use.
SweetSpot’s CDCESs were able to see how Jenny interacted with her PDM (Personal Diabetes Manager), identifying areas where her understanding of the technology could be strengthened.
The SweetSpot Intervention: Personalized Diabetes Education and Pump Training Support
Jenny worked closely with a SweetSpot CDCES who focused on building her knowledge and confidence.
Together, they covered:
The different Omnipod 5 Modes and how to stay in Automated Mode
How to troubleshoot “Limited Mode”
Proper use of the bolus calculator and the importance of pre-meal bolusing
Adjusting Dexcom alert settings for better in-the-moment support
When and how to safely dose correction boluses
At first, Jenny was hesitant to make any settings changes over the phone. She also lacked confidence in her abilities to navigate making the changes as well. But with guidance and step-by-step support, she began making adjustments — and saw results quickly.
Clinical Outcomes: Improved Time in Range and Confidence
Within just a few weeks of consistent support, Jenny’s blood sugar levels began to improve. She started pre-bolusing regularly, spent more time in Automated Mode, and became more engaged in her own care.
Her growing confidence was the real win. Where she once second-guessed her decisions or avoided using certain pump features, she now embraced them.
Jenny’s Dexcom and Omnipod data show a steady improvement over the course of four months:
Automated Mode usage increased from 72% to 100%
Time in Range rose from 52–53% to over 60%
GRI dropped from 55 to 41, meaning fewer dangerous highs and lows
She began actively using the bolus calculator and setting personal goals
Even when Jenny missed something — like realizing her pump had been in Manual Mode for two days — she learned to troubleshoot independently. That kind of self-sufficiency is what remote support is all about.
Why Remote Diabetes Support Works for CGM and Pump Users
Jenny’s transformation didn’t come from more medications or an emergency appointment. It came from being seen, supported, and taught how to use the tools she already had.
By connecting regularly with a knowledgeable and empathetic CDCES, she built the confidence to adjust settings, troubleshoot problems, and engage with her care plan. That’s the power of SweetSpot’s model: We provide frequent, meaningful touchpoints so patients aren’t left guessing between visits.
Empower Your Patients to Succeed with CGM and Insulin Pump Support
Jenny’s story is just one example of what’s possible when patients get the right support at the right time. By combining CGM/pump data review with human connection, SweetSpot helps patients like Jenny feel informed and empowered.
