Sweet Success Story: How Monthly Check-Ins Helped Trevor Jump from 35% to 75% Time in Range

When Trevor, a 70-year-old man living with Type 2 Diabetes, joined his provider’s SweetSpot program, he didn’t know what to expect. Once I connected with him and began regular check-ins, it became clear that just a few simple changes paired with steady, ongoing support could make a big difference. Together, we turned small steps into real progress. 


Finding the First Connection 


In my role as a SweetSpot Clinical Diabetes Specialist, I guide patients to use their CGM (continuous glucose monitor) and insulin pump insights to make meaningful changes. With steady support and small adjustments, progress comes naturally, just like it did for Trevor.  


When I first reviewed the data from Trevor’s CGM, his Time in Range was only 35%. His blood glucose stayed high most nights, often spiking after late dinners and evening snacks. I saw from previous notes that Trevor’s provider had recommended that he take Humalog with meals, but I suspected this wasn’t happening. 


When I spoke to Trevor, our brief conversation revealed a lot: During the day, Trevor’s glucose stayed fairly steady from low-carb snacking, but his large evening meals caused extended overnight hyperglycemia. 


I focused on one clear message: Start giving Humalog before dinner, not hours later. I also offered small dietary suggestions, like adding protein first and limiting late-night carbs. Trevor agreed to give it a try, even though I could sense he was hesitant. 


A Breakthrough in Understanding 


By his second review, Trevor’s engagement had shifted. His Time in Range had improved slightly to 45%, but more importantly, he was curious about the SweetSpot program, wanting to know how it worked and how I could continue to help him. 


That conversation was when I uncovered a key barrier: Trevor believed Humalog was an emergency medication. He thought it was only needed when blood sugars were dangerously high. 


I patiently reframed his understanding, explaining that Humalog is a tool to help prevent highs before they happen. That single word, “tool,” seemed to click. It gave Trevor a new mindset and helped him see that insulin could be proactive, not reactive. 


Staying the Course  


The next month, Trevor’s Time in Range held steady at 45%. He admitted that while he’d used Humalog before meals for a few days, he had slipped back into old habits. Still, something was different this time: he was thinking about change. 


Trevor began paying more attention to his diet, cutting back on carbs and focusing on lean proteins and vegetables. He wanted to lose weight but didn’t want to feel deprived, so I worked with him to brainstorm dinner options that were both healthy and satisfying. 

 

Turning Point: Consistency Wins  


Two months later, the payoff was clear. Trevor’s Time in Range had climbed to 75%, and his overnight glucose patterns had transformed. His highs were milder and shorter, and on many days, his glucose stayed perfectly in range all night. 


Trevor credited two changes: Giving Humalog with dinner and making better food choices.  


I celebrated his progress with him, reinforcing the habits that had worked and helping him identify new, realistic goals to keep improving.


Lessons in Long-Term Change 


Looking back, I credit Trevor’s success to steady, consistent support. 


Trevor’s provider had been reinforcing the right guidance for years during their regular visits. What truly helped bring that advice to life, however, was the ongoing connection that SweetSpot provides.
