Sweet Success Story: How CGM Monitoring and Education Changed Michael’s Diabetes Journey

When Michael, a 60-year-old man newly diagnosed with Type 1 Diabetes, began struggling with wide glucose fluctuations and persistent hyperglycemia, the issue was not a lack of motivation. It was a lack of education and support.
Michael developed Type 1 Diabetes because of immunotherapy treatment during his cancer care. Like many patients diagnosed later in life, he was suddenly navigating insulin therapy, injections, timing, and glucose management with very little understanding of how it all worked together.

Identifying the Problem

Time spent in the “very high” glucose range dropped from 30% to 6%
But the impact extended beyond the numbers.
Michael reported that he was:

Sleeping better at night
Taking less mealtime insulin
Feeling significantly more confident managing his diabetes
He also shared that this was the most thorough conversation he had ever had about his diabetes management.
What made the biggest difference in Michael’s case was individualized education between office visits.
Rather than simply reviewing numbers, I made sure to focus on understanding Michael’s concerns, daily routines, and decision-making process. By walking through real-world examples together, he was able to connect insulin actions to his glucose outcomes in a way that finally made sense.
This case also highlights an important reality in diabetes care: many patients leave appointments without fully understanding how to safely and effectively use insulin therapy, especially when diagnosed later in life or managing multiple health conditions simultaneously.
