Proactive Diabetes Care That Works: Real-World Outcomes from SweetSpot

New data shows measurable improvements in diabetes outcomes for patients enrolled in SweetSpot’s remote diabetes monitoring program.

Continuous glucose monitors (CGMs) and insulin pumps provide powerful insights into a patient’s glycemic health. But data alone doesn’t improve outcomes — it must be reviewed consistently and acted on between visits. Many endocrinology practices struggle here. Device data is scattered across platforms, and already-busy care teams can’t always deliver the proactive support patients need. 


SweetSpot solves this challenge by combining a centralized data dashboard with a dedicated team of Certified Diabetes Care and Education Specialists (CDCESs). Acting as an extension of each clinic, our team performs monthly data reviews for each patient, reaches out to patients between visits when clinically necessary, and provides timely treatments recommendations to patients (as approved by providers). 


The result? Better outcomes and support for patients and less burden on practices. 


Results in Just Three Months 


We recently analyzed outcomes for patients enrolled in SweetSpot at a partner clinic and found measurable gains within the first three months. Using the Glycemic Risk Index (GRI), a validated score that balances hyperglycemia and hypoglycemia risk, we tracked improvements across patient groups. 


The GRI is an aggregate score that is calculated based on key CGM metrics. Patients can fall into 5 possible GRI zones (A-E), with Zone A equating to ideal control. For more detail on GRI, see here


We found that patients who started in higher-risk GRI zones showed significant improvement in their risk levels after three months enrolled in SweetSpot:

 

  •  70% of patients who began in Zone E improved to lower risk zones. 

    • This includes 20% of patients who improved to either Zone A or Zone B.  

  • 75% of patients who began in Zone D improved to lower risk zones. 

    • This includes 25% of patients who improved to Zone A, and 50% of patients who improved to Zone B.

For patients who started in lower-risk zones (Zones A & B), clinical stability also improved:


  • Zone B: 80% of patients either remained in Zone B or improved to Zone A with our clinical support. 

  • Zone A: 94% of patients remained stable in Zone A with our clinical support. 


These results highlight a key takeaway: consistent data reviews and timely interventions drive meaningful change. Without dedicated support, many of these high-risk patients would have remained unmanaged, and stable patients might have drifted into higher risk.
